HSSP English Parental Consent Form

We, consent to our child’s participation in the High School Summer Program (hereinafter referred to as the “Program”) of the Center for Hellenic Studies, Harvard University (hereinafter referred to as “CHS Greece”) and our child's presence in the CHS Greece Iatrou building in Nafplio and in other buildings in Nafplio where the Program takes place, from July 2 to 17, 2020.

We understand that CHS Greece has the right to photograph, video/audio-record, and interview our child during the Program, and to use, according to its judgement, whole or parts of the oral and written word of our child; his/her performance, image, name, face, voice, and other characteristics of our child (his/her overall presence during the Program), for the evaluation, improvement, and/or promotion of the Program.

We agree to transfer to CHS Greece all rights for using these photographs, recordings (video, audio, etc.) and we also consent that CHS Greece has all rights to use our child’s written answers to questions posed during the Program. We understand that our child’s name, language, presence, voice, and image can be publicized through the Center’s website and/or though websites of other collaborative institutions (online or though other media platforms) for further improvement and promotion of the Program.

We consent to the possibility of CHS Greece recording our child’s performance and to use and re-use, according to its judgement, whole or parts of this recording for promoting its programs, and to authorize other parties to do the same. Furthermore, we recognize that the Center bears no responsibility for the re-publication in any possible way of the above-mentioned recordings or any part of it that may be illegally made by any third party, by downloading them from the internet. We also recognize that although the image or video material is intended for digital and/or printed use by CHS Greece, there is no guarantee by CHS Greece and no claim on my side that there will be total or partial appearance of that material in electronic or printed means of CHS Greece.

We understand that CHS Greece can adjust, change, edit, add, delete, combine with other material, the recordings, reproduction, and distribution of the material, or else to use our child’s performance in the Program with any means/tools/media known today or in the future.

We agree that CHS Greece will be the exclusive owner of the intellectual property and all other rights of images, video and/or audio recordings, as well as every other media, in which our child appears. We consent that neither we nor our child will be entitled to any use or compensation, now or in the future, in connection with these rights.

We understand that the Center’s employees and associates have the right to access and process the above data, as well as individuals/companies that support their operation (accounting office, maintenance of IT systems and applications), for the sole purpose of dealing with the object of their work.

The rights provided above are indefinite and irrevocable and we understand that the CHS Greece will act in accordance with this form. We declare that we have no claim or requirements from Center resulting from or in connection with the above. This participant parental consent form is governed by Greek law and is subject to the jurisdiction of the courts of Athens, Greece.

CHS Greece takes all necessary measures to ensure the security of the personal data of the participants of its activities.

With respect to your child’s personal data, you have the following rights:

• Right of data access: The right to know if data is being processed, how, and for what purpose.
• Right of data rectification: The right to ask for a rectification of your child’s personal data if it is inaccurate or incomplete.
• Right of data deletion (“right to be forgotten”): The right to request the deletion or removal of personal data under certain conditions.
• Right to restriction of data processing: The right to request the restriction of the processing of personal data when certain conditions are met.
• Right to data portability: Your right to ask to send your child’s information to a third party (e.g. another support center) when certain conditions are met.
• Right to object to processing: The right to object to the processing of personal data. The Center will no longer submit data for processing unless there are legitimate reasons for processing or for the foundation, exercise or support of legal claims.
• When you submit a request by exercising one of the above rights, the Center must respond within one month, or later, but explain the reasons for the delay.

Transportation/travel, accommodation, meals, and other expenses

We acknowledge that the CHS Greece has no responsibility and/or obligation for our child’s transportation/travel to and from the location of the Program (Nafplio, Greece), and we declare that we take full responsibility of that transportation/travel and its cost.

We acknowledge that CHS Greece does not bear any responsibility and/or obligation, except for special cases that may be decided and communicated by the Center, for the living arrangements of our child at the Program location, and we take full responsibility for this selection and overall accommodation arrangements and their cost. We also acknowledge that CHS Greece does not bear any responsibility and/or obligation for our child’s travel both to and from the Iatrou building or to and from other buildings where the program takes place during the Program, except for special cases that may be decided and communicated by the Center, as well as for any possible travel during the Program’s daily breaks. In the context of the day-to-day program of seminars, discussions, etc., we allow our child to take lunch breaks outside the Program’s locations, and we declare that during the time that our child is outside of the buildings where the program takes place, CHS Greece bears no responsibility for any of his/her activities.

We also take full responsibility for any other independent travel of our child before, during, or after the closing of the Program. We understand that CHS Greece does not encourage, and may limit or restrict the independent travel of our child during the Program, and we agree that, if we wish our child to travel independently, we must submit an itinerary to the Program leaders and managers for advance approval in order to preserve the smooth operation of the Program.

We acknowledge that we are responsible for all arrangements and costs related to travel, accommodation, meals, and other incidentals related to our child’s participation in the Program, beyond those that may be covered by CHS Greece, as well as any additional expenses that our child may incur during the Program.

* We understand that our child’s voluntary participation in the Program may involve risks different than the ones found within the country of his/her residence. These include, but are not limited to, risks involved in traveling to, from and within international destinations; foreign political, legal, medical, social, and economic conditions; different standards of design, safety, and maintenance of buildings, public places, and conveyances; different local weather conditions, eating habits, etc. Although Greece overall, exercises normal safety precautions, we understand that this country has its own health and safety standards that may vary from those in the country of our child’s residence and we declare that we have read and understood all these differences taking full responsibility for any related problems that may occur. We have also reviewed all relevant information provided by the Ministries of Foreign Affairs and Health of the country of our child’s residence, as well as health advisory information available from the World Health Organization related to Greece and travel abroad.

We understand that, although the organizers have planned the Program in the best possible way, they cannot eliminate all the risks related to our child’s travel/transportation, accommodation, safety of his/her personal belongings, etc.

Standards of participation

We declare that our child will fully conduct him-/her-self in a manner compatible with local laws and regulations; with the Center’s regulations for visitors; with policies for participant conduct set forth in this document, in any Program-related materials and as communicated by the Program leader and coordinators. Our child will refrain from improper or offensive conduct, or otherwise inappropriate behaviors that include, but are not limited to, disorderly behavior, sexual harassment or misconduct, use of drugs, or alcohol consumption. We understand that if at any time during the Program our child fails to honor this agreement, then organizers may, in their sole discretion, immediately dismiss or withdraw our child from the Program.

We understand that we are fully responsible for making sure that our child has valid and current health/medical insurance and carries a valid insurance identity card with him/her during the Program. We have reviewed our child’s coverage and have determined that his/her health/medical insurance is adequate to cover injuries or illnesses, etc., that our child may sustain while traveling abroad. We will be solely responsible for payment in full of all costs of medical care our child may receive during the Program. We further agree that CHS Greece has no responsibility whatsoever for any health/medical issues of our child that may occur during the program, as well as any actions taken by the organizers to provide necessary emergency medical care to our child, and we authorize CHS GR to contact us during any emergency situation.
