Performing Wisdom | Application

Please fill out the application completely before you click on the “Submit” button at the end of the form. The system will not save incomplete applications. If you hit the “Enter” key before completing the application, the application may submit accidentally. If this happens, simply begin a new application. Any incomplete duplicate application forms will be discarded by our processing team.

For the essay questions that require more lengthy responses, draft your answers in a separate application, such as Microsoft Word, and have them ready to paste into the application form.

Parental Consent Form

Please ask your parent(s)/guardian(s) to complete the Parental Consent Form by Friday, March 13, 2020 at midnight.

Deadline

The deadline for all submissions is Friday, March 13, 2020 at midnight. Once you have completed your application, the CHS staff will contact your references and ask them to fill out a brief questionnaire by Tuesday, March 17, 2020.

Reference

Please provide the contact information for a reference who can speak to your academic work and evaluate your plans for participating in the program. If you cannot supply the contact information for a teacher at your high school, please provide information for an employer or mentor who know you well.The CHS will contact your reference directly with a questionnaire. Be sure to inform your reference about your application. The sooner you submit your application, the earlier the CHS will contact your reference.
